First Visit

Becoming a Patient

Thanks for your interest in Dr. Johnson’s preventive care dental clinic. We’d like to invite you to a pre-visit office tour to see our modern, “green” solar heated facility, see our prairie, and meet Dr. Johnson and his experienced team of dental professionals. We’re located a few minutes north of Fond du Lac’s Forest Mall.

Registration

Of course you will need to do some paperwork, registration form, health history form, and HIPAA Release. We can send them, you may pick them up at your office tour, or for your convenience, you may download, print, and complete them ahead of your visit by clicking on the links above.

Emergency Care

If you have an emergency, emergency concern, or request a 2nd opinion before your scheduled comprehensive examination, please call (920)923-0310. The emergency visit will be problem focused (Dr. Johnson will examine one tooth/area) and give recommendations for treatment. If the problem is treatable in the allotted time available, Dr. Johnson will offer treatment. Or, if extensive, we’ll reappoint. Our goal is initial pain control, giving you some insight as to treatment options.

Comprehensive Examination

The comprehensive examination will require approximately 1 ½ hours, so plan accordingly to be fair to yourself. This exam is quite thorough and will consist of:

  1. Necessary radiographic X-rays
  2. Periodontal Examination
  3. Dental/tooth Evaluation
  4. Intra-oral camera tour of your mouth
  5. Oral cancer screening
  6. Consultation of Findings

If there is minimal discovery of disease or dental problems, recommendations will be discussed at the first visit. If more information and records are required, a separate sit-down discussion will be recommended. The consultation will include ideas for dental restoration, smile enhancement or makeover, recommendations for further treatment options, and financial options for your dental investment.

What Next?

Periodontal health is paramount to any dental success. There must be a good foundation to your dental health. “Perio” means around, “dontal” means tooth. Periodontal health refers to the health of the gums (gingiva) and bone support around the tooth. We will empower you to be able to maintain and protect your dental investment, or all will fail.

Some expect a “tooth cleaning” (dental prophylaxis), however, be careful to know exactly what entails a tooth cleaning, what do you get, do you have the tools necessary to maintain your teeth clean? If not, a tooth cleaning is only good for 3 days! The answer lies in your periodontal collars; which is diagnosed by pocket-plotting.

Child’s First Visit

We generally see children once they have all their teeth, and are adaptable to new environments. Dr. Johnson recommends morning appointments, as this is often their best time of day (ours too!). On the first visit, we will do as much as they’re comfortable with, sometimes it’s just a ride in the chair---that’s ok! We want their visits to the dental office to be a good experience! (Better than Dr. Johnson’s first visit as a youth.)

Insurance Talk

It is good if you have some insurance coverage. Be aware, dental insurance usually covers only a (small) portion of your dental services. It is also fortunate to know it is always your choice in what treatment you wish to pursue. You and your health are important to us. We make every effort to base sound treatment options on what we feel is best for you, what's in your best interest, NOT based on what your insurance benefits will cover. That is why we take extra time to inform you of your treatment options, insurance benefits, and insurance limitations. 

We accept most insurance coverages. If you desire to know what benefits are available, please completely read your insurance booklet, then we can submit to your insurance company for determination of benefits. The insurance company will tell you what it will pay, what is your co-pay, what is covered, what is not covered (however, you decide what treatment you desire.)  Be aware, there are better insurance companies than others, and ultimately your coverage will usually be dependent upon what your employer negotiated with a particular insurance company.

Flex Spend

These tax-savings, pre-tax, set-aside savings give you greater options with treatment as you decide your investment. We feel the best investment you can make is into your health. Research your Flex-Spend guidelines, and then we can discuss your best way to proceed for your maximum benefits. 

Financial Discussion

Following your comprehensive examination, diagnosis and treatment options, you will also have options for payment.

Our receptionist will discuss options including Care Credit. Care Credit, when qualified, allows you to finance up to 24 months without interest, giving  you opportunity to ideally restore your dental health without further delay and without deterioration due to the continued progress of decay or periodontal disease. A your disease progresses, the treatment becomes more extensive, and likely, more expensive.
